چکیده

Introduction Early repolarization (ER) is a common electrocardiographic finding and is diagnosed by the presence of a J-wave amplitude of Z0.1 mV in Z2 contiguous inferior or lateral leads of a standard 12-lead electrocardiogram (ECG), which is manifested as QRS slurring or notching. The ER pattern is observed in 1%–13% of cases in the general population and is mainly thought to be benign. However, Haïssaguerre et al have underlined the potential malignant role of an ER pattern in idiopathic ventricular fibrillation (VF) patients. Of 206 idiopathic VF cases and 412 controls, ER pattern was more frequently present in patients with idiopathic VF than in those in the control group (31% vs 5%). In addition, a population-based study from Finland including 10.864 middle-aged people found an ER pattern in 630 individuals leading to an increased risk of cardiac death. An ER pattern on the ECG in patients resuscitated from VF is called early repolarization syndrome (ErS). So far, variants in several ion-channel genes have been associated with ErS. Variants in KCNJ8 are associated with a gain of function with an increased outward potassium current, and variants in the subunits of the cardiac L-type calcium channel (CACNA1C, CACNB2, CACNA2D1) lead to a reduced inward calcium current. Finally, a variant in SCN5A causes a loss of function of the sodium channel, which leads to decreased inward sodium current.

Case report: electrical storm during induced hypothermia in a patient with early repolarization

BACKGROUND Population based studies showed an association of early repolarization in the electrocardiogram (ECG) and a higher rate of sudden cardiac death presumably due to ventricular fibrillation. The triggers for ventricular fibrillation in patients with early repolarization are not fully understood. CASE PRESENTATION We describe the case of a young patient with a survived ventricular fibr...

Letter by Casado-Arroyo et al regarding article, "Electrocardiographic characteristics and SCN5A mutations in idiopathic ventricular fibrillation associated with early repolarization".

BACKGROUND Recently, we and others reported that early repolarization (J wave) is associated with idiopathic ventricular fibrillation. However, its clinical and genetic characteristics are unclear. METHODS AND RESULTS This study included 50 patients (44 men; age, 45 ± 17 years) with idiopathic ventricular fibrillation associated with early repolarization, and 250 age- and sex-matched healthy ...

Sudden cardiac arrest associated with early repolarization.

BACKGROUND Early repolarization is a common electrocardiographic finding that is generally considered to be benign. Its potential to cause cardiac arrhythmias has been hypothesized from experimental studies, but it is not known whether there is a clinical association with sudden cardiac arrest. METHODS We reviewed data from 206 case subjects at 22 centers who were resuscitated after cardiac a...

Radiofrequency catheter ablation for treatment of premature ventricular contractions triggering ventricular fibrillation from the right ventricular outflow tract in a patient with early repolarization syndrome

Introduction Ventricular fibrillation (VF) triggered by premature ventricular contraction (PVC) most frequently originates from the His-Purkinje system and right ventricular outflow tract (RVOT) in patients with idiopathic VF. Radiofrequency catheter ablation (RFCA) of triggering PVCs is reportedly useful for the successful elimination of VF. The diagnosis of early repolarization (ER) syndrome ...

Early repolarization associated with ventricular arrhythmias in patients with chronic coronary artery disease.

BACKGROUND Early repolarization, indicated on the standard 12-lead ECG, has recently been associated with idiopathic ventricular fibrillation in patients without structural heart disease. It is unknown whether there is an association between early repolarization and ventricular arrhythmias in the coronary artery disease (CAD) population. METHODS AND RESULTS Patients with CAD with implantable ...
